Tier D: Chair massage (minimum of one (1) hour maximum of three (3) hours) (approximately 4 hours of total time) Each massage 15 minutes in duration, maximum of 12 sessions.

Three hours $150.00 up to 12 sessions
Terms and Conditions:
Halcyon Holidays will provide all necessary equipment including, but not limited to a massage table or chair, sheets, face cradle covers, and oils or creams. Customer will provide an adequate open space for the table or chair with room for the therapist to provide the massage.
All individuals who are to receive a massage shall sign a request and informed consent form, and shall complete such intake history as may be requested by the therapist.
MINORS--No massage services shall be provided to any individual under the age of fourteen (14). For minors ages fourteen to sixteen (14-16), a parent must be present and sign a parental consent form. For minors age seventeen, only a signed parental consent form is required.
DEPOSIT-- A deposit of fifty percent (50%) is required. A reservation agreement is not complete until Halcyon Holidays receives the deposit. If the party is canceled at least ten (10) calendar days prior to the scheduled date, then half of the deposit will be refunded. If a party is canceled less than ten (10) days prior to the scheduled date, the deposit will not be refunded.
CHANGE OF DATES-- With at least ten (10) calendar days’ notice, a party’s scheduled date may be moved to an alternate mutually agreeable date, selected from one of three dates that Halcyon Holidays shall offer and make available to Customer, and scheduled within ninety (90) days of the original schedule date. With less than ten (10) calendar days’ notice, a ten dollar ($10.00) fee will apply. If Customer does not choose one of the three alternate dates offered by Halcyon Holidays, then the party shall be deemed canceled.
Payment of the outstanding balance is due on the date of the party. Halcyon Holidays accepts cash or checks, but does not accept credit cards. For Customer’s convenience, payment may be made by PayPal, but there is an additional five dollar ($5.00) fee.
